How To Apply

STEP ONE – LETTER OF INTENT

DEADLINE: TUESDAY, JUNE 1, 2010 5:00 P.M.

Submit 5 copies of the Letter of Intent Cover Page and 5 copies of your answers to the Letter of Intent Narrative Questions. Use 12-point font with 1-inch margins and use the numbers as headings for your responses. NO double sided copies, binders, cover folders or staples.

Proposal material should be presented in the following order:

  1. Letter of Intent Cover Page, filled out in full, including signatures from the Executive Director and chief officer of the Board of Directors. 
  2. Narrative Questions attachment, please limit responses to all questions to no more than five combined pages.
  3. Fiscal Sponsor Information Form (if necessary).
  4. One copy of your organization’s IRS 501 (c) (3) determination letter.

APPLICANTS SELECTED FOR FURTHER CONSIDERATION ONLY

STEP TWO – SITE VISIT/ORGANIZATION INFORMATION

DEADLINE: FRIDAY, AUGUST 13, 2010 5:00 P.M.

Submit 1 copy of the Site Visit/Organization Information Cover Page and 1 copy to the answers to the Financial Information section. NO double sided copies, binders, cover folders or staples.

Proposal material should be presented in the following order:

  1. Site Visit/Organization Information Cover Page, filled out in full, including answers to the Financial Information section.
  2. One copy of each of the following attachments:
  • Current Board of Directors list
  • Audited Financial Statements (if available)
  • Most recent IRS Form 990
  • Current fiscal year operating budget
